Certificate Issuance （Nursing Care JLCT）
for submission to the Immigration Service Agency, Organization for Technical Intern Training, etc.
We only take orders from educational institutions, not from individual examinees.
IMPORTANT
- This certificate is an important document for examinees. Please obtain the approval of the examinee in advance.
- We, JLCT will not be liable for any damages arising from or in connection with this certificate.
- Certificate issuance will take approximately 4 weeks following the test date.

- Cancellation will not be accepted after the certification has already been sent out.
How to receive the certificates

STEP 2
Order & Confirmation call
Complete the order form andFAX it to 03-3239-7585
.
Please carefully fill in the form to ensure that the details are consistent with those registered on the examination.
If any information is missing or incomplete, the processing time will be longer.
After sending the fax, please call the JLCT office (03-3261-1212) to confirm that it has arrived.
STEP 3
Certificate issuance and delivery
After confirming that the information is complete and correct, a certificate will be issued.
If the form is incomplete, we will contact you by telephone to request that the necessary corrections be made.
Certification will be issued and shipped within 3 business days of the submission of the order form(date of correction completion).
Cancellation will not be accepted after the certification has already been sent out.

Receiving the certificate

When receiving your certificate, please have
the issuance fee (500 yen per certificate)+ cash on delivery service fee＜290yen＞ + postage ＜180～750YEN, Varying depending on the weight＞ ready.(As of June, 2024)

When the details in the order form and our record don’t match
When the details(examinee’s name, birth date, etc) in the order form and our record don’t match, certificates can not be issued.
We will call you to confirm the details and inform you of the following procedure.
【When the order form is INCORRECT】
Please fill in the form correctly, write “Revised” at the top right, and FAX it again.
【When the order form is CORRECT】
The original test record has to be updated.
The examinee must ask the local host institution to make a correction.
The examinee will be asked to show an official picture ID and to pay for reissue fee.
After the corrections have been made, the examinee can order the certification.
- Examinees in Japan can request corrections to the institutions(nursing care facilities and supervising organizations, etc) or directly contact JLCT.
- Depending on the revision request, we may not be able to accept the order. Therefore please take that into consideration.
